If you are a current Cone Health patient using myCone Health, you will need to activate your Cone Health MyChart Login to continue to receive access to your ongoing medical records and to communicate with your provider.
If your primary care physician is with a different organization, please continue to access your non-Hoag-related records through their existing systems/portals.

Only you, your provider, and your healthcare team are permitted to view the information sent through MyChart. Cone Health implements security measures designed to protect your information from unauthorized access.
Your account password protects your account and we urge you to take steps to keep your personal information safe by not disclosing your password and by logging out of your account after each use.
Cone Health further protects your information from potential security breaches by implementing certain technological security measures including encryption, firewalls, and secure socket layer technology. However, these measures do not guarantee that your information will not be accessed, disclosed, altered, or destroyed by a breach of such firewalls and secure server software.
In addition, Cone Health MyChart login access requires a MyChart unique username and password. The Cone Health MyChart is owned and operated by Cone Health and is fully compliant with federal and state laws about your privacy. Your name and email address will be kept private and confidential and will never be sold or released by them.
If you learn or suspect that your username or password has been wrongfully used or disclosed, you should promptly notify us and immediately reset your password. To help ensure the security of your password or account, please sign out of your account at the end of each session.
Hoag Connect MyChart Login
The Cone Health MyChart Login portal is available 24 hours a day, seven days a week for patients. Each patient will be given their user account credentials through which all the health information can be accessed and managed.

If you’re having trouble logging in, click the “Forgot Username?” or “Forgot Password?” link below the log in fields for assistance. You will go through two-step verification to verify your identity so you can recover your username or password.
How to Download the Cone Health MyChart App
Cone is pleased to announce we are now offering you access to your medical records through Cone Connect MyChart. MyChart puts your health information in the palm of your hand and helps you conveniently manage care for yourself and your family members.
You can access your Cone Health account, by downloading the Epic MyChart app and selecting Cone Health as your organization. Go to the Cone MyChart App page to download it for your Android or iOS.
